Self-Neglect and Hoarding Risk Assessment Checklist

The Cornwall and Isles of Scilly Safeguarding Adults Board's Self-Neglect and Hoarding Risk Assessment Checklist is a multi-agency tool designed to support practitioners in identifying, assessing and managing risks associated with self-neglect and hoarding behaviours. It provides a structured and consistent approach to assessing an adult's wellbeing, safety, independence and vulnerability.

The assessment considers a range of key areas, including physical and mental health, nutrition, personal hygiene, home environment, medication, continence and financial wellbeing. Using a graded risk framework, it helps practitioners determine the level of concern, record professional judgement, and identify where intervention or additional support may be required.

The tool supports holistic, evidence-based decision-making and encourages effective multi-agency collaboration when risks are complex or escalating. It is intended to complement professional judgement and should be used alongside safeguarding, self-neglect, mental capacity and risk management procedures.

Overall, the checklist helps practitioners identify concerns at an early stage, promote consistent risk assessment practice, and ensure adults experiencing self-neglect receive timely and proportionate support to reduce harm and improve wellbeing.
