National Safeguarding Adult Review (SAR) Themes and Developments

Safeguarding Adult Reviews (SARs) provide critical insight into how agencies work together to protect adults at risk. National analysis helps identify recurring patterns and systemic issues beyond individual cases.

Second National Analysis of SARs – Key Learning

Professor Michael Preston-Shoot’s Second National Analysis of SARs (2019–2023) identifies priorities for sector-led improvement based on over 650 SARs across England.

Forthcoming National Developments - Baroness Casey Review (Adult Safeguarding System)

Baroness Louise Casey is leading an independent national commission into adult social care, commissioned by the UK Government in 2025. The review is examining how the system operates and what reforms are needed to ensure people are properly supported and protected.

Her work is being delivered in phases, with early findings (March 2026) already highlighting significant concerns about safeguarding, accountability and system fragmentation.

Key Messages from Baroness Casey’s Review

From her national speech and early findings, several critical themes have emerged:

What this means for safeguarding

The Casey Review reinforces national SAR learning by:

  • Highlighting system-wide failure, not individual practice failure
  • Strengthening the case for:
    • national oversight of SARs
    • stronger multi-agency accountability
  • Emphasising the need to:
    • connect information
    • recognise patterns
    • prevent repeat harm

National Safeguarding Networks

National safeguarding networks provide important opportunities to share learning, strengthen leadership, and promote consistency in safeguarding practice across England. These networks support Safeguarding Adults Boards to work collaboratively, influence policy, and ensure learning from Safeguarding Adult Reviews is applied at both local and national levels.

Both the Independent Chair and the Business Manager of Cornwall and Isles of Scilly Safeguarding Adults Board are active members of these national networks, ensuring that local practice is informed by national learning and that Cornwall contributes to sector-wide improvement.
