A collaborative, person-centred, trauma-responsive approach to support people with multiple vulnerabilities. 

My Team Around Me (MTAM) is designed for those who need sustained, empowering support and better access to services.

As needs become more complex and long-term, creative solutions are required. My Team Around Me provides a multi-agency response in a compassionate and coordinated way. It strengthens collaboration, reduces isolation for both individuals and practitioners and improves outcomes across the system.

Where We Are Now

The Guidance Document is now live and MDTs are conducted under the My Team Around Me approach. This will help build shared practice, consistency, and stronger collaboration across the system.
